The Questionnaire

Before the program we really want you to complete the questionnaire and send it to us. We know it is a pain in the proverbial to fill out. However, what it enables us to do is design the program for you personally rather than give you something off the shelf that a thousand other students have been through exactly. If you are lazy about this, we can't help you they way we would like. If you put all of who you are (in whatever form makes sense to you) into answering the questions, then you have our word, you will be listened to very carefully.

A Brief History: Can you give a brief back ground on yourself to date! What you include/exclude is entirely up to you; you can go back to birth, you can talk about important teachers, family, peers, mentors; you can talk about any jobs you have had so far; you can talk about whatever seems appropriate. The only challenge: do it in one and a half pages!

What do you consider are your greatest ‘wins’ so far: things that have been great successes in your life so far; again they can be from your schooling, your family, leisure activities, sport; you name it - how you answer is up to you.

What do you consider are your greatest ‘muck ups’ or, more appropriately, ‘learning’ opportunities to date; things that haven't worked out as you wished, thwarted dreams, frustrated outcomes, embarrassing moments; they can be private or professional, sport or leisure, school related or about home; the choice of inclusion/exclusion is entirely yours.
